After many years of living in unsuitable housing and feeling unhappy, Jake has finally moved into a home that meets his needs, a place that gives him independence, comfort, and the right level of support. Now he lives on his own, and the transformation has been remarkable. With a peaceful environment, choice in his daily life, and a supportive care team, Jake is now thriving in ways that once felt out of reach.
Jonathan Ashley, scheme manager at Rialto Care and his mother share their reflections on how far he has come.
“For so long, Jake lived in housing that was not suitable for him. Since moving into his new home, everything has changed. He now lives on his own terms, choosing who visits, enjoying peace and quiet of living on his own, and having full control over his space.
We have watched him grow in confidence, start attending events that we arrange, meeting new people, and making choices about what he wants to do and where he wants to go. This year has brought a special moment too. For the first time, Jake bought his own Christmas tree, chose the baubles and tinsel himself, and decorated it with pride.
Seeing him flourish in the right environment reminds us why personalised support truly matters.”
Debbie, Jake’s Mother said, “As his mum, seeing my son happy again fills me with joy. His personality is shining through more every day. I used to struggle to visit him because I don’t drive, and getting to where he lived before took such a long time. Now, it’s just a simple bus journey, and that means the world to me.
I can see how much has changed now he’s living in his own home. Where he was living before, there were so many people and he would often not be wearing his own clothes. This has changed. He’s wearing his own clothes again, and enjoying life in ways I haven’t seen for years. The care team supporting him are fantastic, they truly understand him and I know he’s finally getting the right support.
I’m excited for his future now. For the first time in a long time, I have started seeing my son becoming himself again – we are not there yet, and it’s getting more and more every day. I’m so happy.”
